Prophylactic Internal Fixation of Fibrous Dysplasia in Proximal Femur

Abstract
A pathologic fracture is a fracture of abnormal bone. Typically, the fracture occurs during normal activity or with minor trauma. Osteoporosis is the most common cause, and bone tumor is the second most common pathologic condition associated with pathologic fracture. The proximal femur is the most common site of pathologic fracture because of its weight-bearing. Because pathologic fracture of the proximal femur can occur even in a benign bone tumor and can be greatly affect the quality of life. The most common benign bone tumor in proximal femur is a fibrous dysplasia. In the management of the fibrous dysplasia, generally, we have been treated the lesion by curettage and bone grafting. But in large sized(at least 2.5cm diameter) or more cortical involved(at least 50%) lesion, we have tried additional prophylactic internal fixation. It is generally agreed that an impending fracture should be treated with internal fixation before it breaks because the patients do better in all aspects, especially with regard to easier technique and rehabilitation, as well as decreased postoperative complication and mortality rates. There were no evidence of recurrence, pathologic fracture and deformity. Prophylactic surgery of fibrous dysplasia should also be considered, if indicated.
